I've never posted here before though I have lurked quite a bit. I have been having a scale bar problem today I am hoping someone might shed some light on for me.
I am creating a 24" x 36" orthophoto based map that is 1:245, 1 inch= 20 feet. When I print the map out, the scale bar reads 1 inch=20 feet but when I checked the scale bar with an actual engineering scale, it was off slightly- 1 inch actually equaled about 19 feet.
I know that I can potentially fix this by converting the scale bar to graphics and manually resizing it... but is there any other way to fix this problem? I am guessing there is a very simple answer...

It's possibly a print conversion issue. What format are you printing from? If PDF make sure you have the "scale to fit..." option unchecked.
I assume you are printing from a projected layer? If its Lat/Long there may be an issue there as well. If the scale bar is dynamically adjusting to the changing scale of an un-projected image its possible that it may change dimensions a little but not enough to kick the verbal scale over.
Chances are it's neither of these, but all I could think of. Good luck!

Are you by chance creating a pdf from a mapbook? If so, I believe this is a known issue. To fix it you do have to convert the scale bar to a graphic.

I'm printing directly from the mxd file....
Also, it is in NAD 83 state plane ... does that matter?
