Outline of Cost Estimate for Travel / Rec. Map

I have searched Google, and this forum, to no avail. I have a client who is asking my company for a cost estimate. The paperwork I need to develop includes both GIS processing and Illustrator-based cartography for output to paper/synthetic via CMYK. There is minimal to no fieldwork involved. I am a one-person shop, and I have more than four years of experience developing medium-to-high-end GIS and Illustrator-based output. I am in search of a bare bones outline for a cost estimate. Too much detail would not suit the client. In other words, I am thinking of an every-fundamental-cost-estimate-should-have-this list (or outline) for a travel and camping map.

Here's a scrubbed version of a very simple cost estimate I recently submitted for a basic recreation map. Not sure if this is what your after but could be a good jumping off point.
David- Looks great, much thanks for the post. If anyone else replies in the next few days I will summarize responses and add that info into this thread; hopefully that will help out the next person looking for similar advice.

1. Explain exactly, and to the level of detail required, what your understanding is of the client's needs.
2. Make sure you're "covered" for the almost-inevitable need for the client to make changes.
3. Make clear what your policy is relative to use. Is the client allowed unlimited use? Are you yourself allowed to reuse the file for other purposes?
Here's the version I use for American clients (there's another version that I use for non-US, using spelling that's "antiquated" or "correct", depending on your point of view):
Thank you for your request for quotation. Please read the quotation form below carefully. By approving this quotation, you are placing an order with Map Graphics, subject to its conditions.

-- Final file(s), with watermark removed, will be delivered electronically after your approval and payment, in gif or Adobe Illustrator x format. Other formats are available on request. Delivery is by ftp unless requested otherwise.

-- Most maps are illustrative and generalized, and can be presented with varying degrees of detail. If our price is not within your budget, please let us know. Usually we can adapt the graphic to fit any reasonable budget.
-- We do not do "image-mapping"; i.e., hotlinking of graphics. Please instruct your web developer to do this after we have delivered the final graphic.
-- You have full rights to your map(s), except for resale to third parties. We do not charge royalties or user fees. Map Graphics also retains the right to re-use the original graphic files.

Thanks very much to Charles Syrett and David Medeiros for their posts. The two sample cost estimates they provided are useful for different output formats (one for CMYK, the other for the web). Here are, IMHO and from my understanding, the important areas they cover. It also includes an additional bit of my own experience:
[company's best understanding of what the client wants, including a succinct description of styles, color, size, format, map grids, legend, number of individual files (e.g. insets), and north arrow]
[price range for entire project, with caveats about final scale and feature density...includes concise info on optional layers and their associated costs]
Ordering and Delivery
[how the customer can place an order, electronic and physical delivery details, delivery timing details]
[how to deal with client changes, costs of design changes, notes on dealing with and sharing fonts, notes on who will provide map graphics, notes on what to do if data changes, naming convention for delivered files]
[payment methods available and delivery timing for the final product...essentially the final product will be delivered upon approval of the final version, and once payment is received]
Special Notes/Estimate Assumptions
[Information about royalties and copyrights, notes on specific services not provided, special instructions about the involvement of web developers and printers, notes on how to reduce costs if the estimate is too much]
