Do you have Maplex? If so, set your map units to kilometers, then set Maplex setting for road label to remove repeat labels every X kms.

Are you talking about placing "kilometer markers" at specific intervals? If so, look into Linear Referencing in the Toolbox. Or you can try the Divide command in the Editor dropdown for this as well.

If the distances are in the attribute table of your layer, then a quick (and dirty) way to label is just right-click on the layer select properties, and use the label tab to add labels. From there you can convert to annotation and manually change the location of the labels.
