MetaData for Maps

The shop I work in produces a large quantity of maps each and inveitably someone comes around looking for a map that was done a few months or years ago. Occasionally some one asks about the map itself - what data went into it, who approved it, why certain choices were made. Alot of this information we keep with the project folder so usually it isn't too hard to dig up.
Have any of you had any experience standardizing map metadata (not data metadata)? What have you included? How have you gone about collecting this information? How much of this information shows up on the map? I am looking to put together some guidelines on the topic and would welcome your feedback.

At my last job, we implemented metadata standards for documents and images using metadata elements from the Dublin Core. It is a comprehensive metadata standard used for all types of documents. We implemented a simplified version. Our concern being that trying to fill out too many fields who lead to people not putting any information in rather than getting the necessary information.
